Foundayo: FDA-approved, once-daily, and oral

Foundayo received FDA approval effective April 1, 2026, for adults with obesity, or adults with overweight who also have at least one weight-related medical condition such as hypertension, dyslipidemia, obstructive sleep apnea, or cardiovascular disease. It's approved to be used alongside a reduced-calorie diet and increased physical activity. This is currently a weight-management indication only — orforglipron has not yet been approved for type 2 diabetes, though Eli Lilly has indicated a submission for that indication was anticipated.

The approval moved unusually fast: it was granted under the FDA's Commissioner's National Priority Voucher (CNPV) pilot program, and Foundayo was the first new molecular entity approved under that program, clearing just 50 days after filing. Lilly began shipping Foundayo on April 6, 2026 through LillyDirect, with rollout to retail pharmacies and telehealth providers following shortly after.

What sets orforglipron apart from other GLP-1 medications isn't just that it's a pill — Rybelsus is also an oral GLP-1 — it's that Foundayo is described by Lilly as the only GLP-1 pill for weight loss that can be taken any time of day without food or water restrictions. It's a small-molecule, non-peptide tablet, available in six strengths (0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg, and 17.2 mg), swallowed whole. For people who've wanted GLP-1 results without needles or the timing rules that come with some other oral options, that combination is the headline.

What the ATTAIN-1 trial showed

Foundayo's approval was supported by data from two randomized, double-blind, placebo-controlled trials, most prominently the Phase 3 ATTAIN-1 trial (NCT05869903), which enrolled 3,127 adults with obesity or overweight plus a weight-related condition, none of whom had diabetes. Full results were published in The New England Journal of Medicine and presented at EASD 2025. At 72 weeks, all three doses tested — 6 mg, 12 mg, and 36 mg — met the trial's primary endpoint of superior weight reduction compared with placebo.

At the highest dose tested (36 mg), participants who stayed on treatment (the efficacy estimand) lost an average of 27.3 lbs, or 12.4% of body weight, compared with 2.2 lbs (0.9%) on placebo. Looking at everyone assigned to the highest dose regardless of adherence (the treatment-regimen estimand), average weight loss was about 25 lbs (11.1%) versus 5.3 lbs (2.1%) on placebo. Among highest-dose participants in the efficacy analysis, 59.6% lost at least 10% of body weight, and 39.6% lost at least 15%.

The trial also tracked cardiometabolic markers, with improvements in waist circumference, non-HDL cholesterol, triglycerides, and systolic blood pressure, plus a 47.7% reduction in hs-CRP, an inflammation marker, at the highest dose in an exploratory analysis. A companion trial, ATTAIN-2 (NCT05872620), enrolled more than 1,600 participants with obesity or overweight and type 2 diabetes as part of the same registration program supporting the approval.

Common side effects

In ATTAIN-1, the most common adverse events reported with orforglipron were gastrointestinal — nausea, constipation, diarrhea, and vomiting — and were generally described as mild to moderate. That pattern lines up with what's already well documented across the injectable GLP-1 class, like Zepbound and Wegovy, so people switching from an injectable to Foundayo are working with a broadly familiar side-effect profile, even though the delivery method is completely different.

Discontinuation due to adverse events ranged from 5.3% to 10.3% across the three doses tested, compared with 2.7% on placebo.
